If you wish the school staff to administer prescribed medication, it would be appreciated if parents could please download the medication authorisation form, complete and sign.

For reasons of Health and Safety, please bring the prescribed medicine, as stated, into the school office with the signed authorisation form, which will be included in your child's records.

Please ensure that you have read the "Request to Administer Medicine" thoroughly.
